A. DECLARATION BY THE APPLICANT

On signing this application form, I declare that -

• The particulars given in this form are true, accurate and complete to the best of my knowledge and belief;
• I have no pending or concluded criminal proceedings against me;
• I have never been declared as an undischarged insolvent;
• I hereby undertake to comply with the Bye-Laws, directions and guidelines issued or the resolutions passed in accordance with the Bye-Laws of the ICSI ISA.
• I undertake that the information furnished by me is true & complete. If found false or misleading at any stage, my enrolment shall be summarily cancelled;
• I undertake to furnish any additional information as & when called for by ICSI ISA.

B. FEE DETAILS

• One time enrolment fees: Rs. 2,000 plus GST 18% = Rs. 2,360
• Annual fees for membership: Rs. 2,500 plus GST 18% = Rs. 2,950

• Self-attested copy of Aadhar card/ Passport/ Voter ID/ Driving Licence.
• Copy of PAN.
• Self-attested copy of NISM Certificate certifying successful completion of the "NISM-Series-XXIII: Social Auditors Certification Examination".
• Self-attested copy of documents in support of educational qualifications and professional qualifications.
• Self-attested copy of documents demonstrating practice for the relevant period.
• Copy of proof of payment of Enrolment Fee and Annual Fee.

[Note: Membership Certificate would be issued by the competent authority subject to and in accordance/ due compliance with the guidelines/ terms and conditions, if any, that may be prescribed by SEBI in due course.]
